Student Enrollment Information
Registration for South Cobb High School is every Tuesday and Thursday from 8:30 a.m. - 12:30 p.m. in Room 201 of the main building. Please feel free to contact Mrs. Mauricio at (770) 819-2611 ext.024 if you have any questions in regards to registration.

What do I need to enroll my child at South Cobb?
I need to withdraw my child from school. What do I need to do?
There are FIVE (5) main documents required of ALL students prior to being enrolled in the District?
1. Certificate of immunization
As required by Georgia Law 20-2-771: Georgia Form 3231
2.Certificate of vision, hearing, dental and nutrition screening
As required by Georgia Law 20-2-770 : Georgia Form 3300
3. proof of birth date

Birth Certificate

Military ID

Driver's License

Adoption Record

An official school transcript
-
Affidavit of age
-
A religious record authorized by a religious official
4. Proof of residency
TWO items from the list below, only one from each category, are required by the person(s) enrolling the child in the District.

5. Social Security number
A copy of the student's Social Security Card is required for enrollment.
OTHER DOCUMENTS NEEDED FOR ENROLLMENT
6. A valid transcript and a withdrawal form from the student’s previous school is essential for appropriate academic placement. Parents who can not provide a valid transcript and a withdrawal form from the previous school must fill out Form JBC-8 (Authorization to Release Records) to enroll their student on a conditional basis. Any student who is enrolling in the District for the first time in grades seven or higher must provide a discipline record from the school previously attended in order to be granted full admission status. Refer to Form JBC-9 (Disclosures Required for Conditional Admission). Form JBC-9 must be completed at the time of enrollment.
*Please note that fax request may result in a delay of time needed to enroll your student. It may be necessary to return on the next registration day.
7. For those students enrolling in the Special Education Program: the parent/guardian may provide a copy of the student’s latest IEP.

8. Custodial Documents: The District may request proof of custody in situations involving multiple adults claiming control of the same student.

The School May Also:

request proof of custody or custody documentation; or

suggest that the adult with whom the student lives obtain legal guardianship.

request proof of age and name verification; and
-
other information the school deems necessary for proper placement.
Provide the names and phone numbers of persons who may pick up your child if you cannot be reached, and the name and phone number of your child's doctor.

Withdrawls must be done in the Records Office.

The person who enrolled the student MUST be the person who also withdraws the student.

Records will be relased when the Records Clerk recieves a request from the new school. All request will be honered within 48 hours or 2 working days.
What if my student has already enrolled in a new school?
Records will be released when the Records Clerk receives a records request from the new school. All request will be honored within 48 hours or 2 working days.
Students transferring from one CCSD school to another within the District MUST obtain an offical withdrawl from from the previous school. Any student who is enrolling in the District for the first time in grades 7 or higher must provide a certifed copy of his/her academic and discipline record from the school previously attended. Proof of promotion to the 9th grade is aslo required if the student is a first time 9th grader.
